Take a look at Ticketlog
Below are screen shots of Ticketlog, an Intranet based application used by the Tech Support department at the company where I work. I did it myself, hand-coding in Homesite. I used ASP 3, CSS, JavaScript, and HTML, with SQL Server 2000 on the back end. NOTE: The blue blocks have a purpose. I placed them in order to block off any mentions of the names and contact information of individuals and companies that are customers or employees of ARGO.
